STEP 1: Have ready the Notice of Provisional Ballot Hearing Letter to be marked, reviewed and given to voter when finished with process
STEP 2: Election official will explain and fill out the reason on the  provisional ballot envelope for voting a provisional ballot
STEP 3: Have the Voter fill out a Voter Registration Application (if necessary)


REASONS FOR VOTING PROVISIONAL:  To be explained and filled out by Election Official

No Proof provided for Verification of Voter Registration ( Approved Voter ID), Voter refuses to sign Optional Verification of Identity Affirmation Not on Precinct Voter Registration List
  First Time Voter failed to show ID or current ID shown and no additional ID shown
  Marked Absentee or Early on Voter Registration List
  Poll Watcher Challenge
  Court Ordered Voting Extension

STEP 5: Election Official will sign/ witness the Provisional Voter Eligibility Affirmation

 

PROVISIONAL VOTER ELIGIBILITY AFFIRMATION

I, ______________________________, hereby affirm, under penalty of perjury, that the information provided is true and accurate and that to the best of my knowledge I am a registered voter in this precinct and am eligible to vote in this election. I understand that the County Board of Election Commissioners will count my ballot only upon verification of my voter registration status.



STEP 6: Election Official will write name of Voter on List of Provisional Voters
STEP 7: Use the provided ballot with the green stripe and take the Voter to the ExpressVote and pull up the appropriate precinct given by the County Clerk
STEP 8: Have the Voter cast their ballot and then place the ballot in the BALLOT ONLY envelope
STEP 9: Have the Voter place the BALLOT ONLY envelope and Voter Registration Card ( if filled out) in envelope
STEP 10: Review the envelope with the voter, to make sure Voter information is correct
STEP 11: Mark the appropriate reason on the Notice of Provisional Ballot Hearing Letter
STEP 10: Review the Notice of Provisional Ballot Hearing Letter with the Voter and give it to the Voter
STEP 11: Have the Voter place the Provisional Ballot Envelope in the Ballot Bag

POLL WATCHER CHALLENGE

 

If a Poll Watcher challenges a voter, the Poll Watcher completes the following:

  • Name of Voter Challenged
  • Name of Poll Watcher
  • Name of Candidate, Party or Issue the Poll Watcher represents
